Ageing In Place (AIP) Project Support

The Care Team continues to provide support for Ageing in Place (AIP) Project Support with home visits to residents in need of nursing & rehab care. Starting with assessment of needs, the Care Team now make regular visits to the homebound residents to monitor progress of these residents.
