Social Worker
Requirements
- Degree holder in Social Work, Gerontological Counselling, or equivalent
- Non degree holders also considered, provided qualifications in social work or counselling diploma related courses achieved and couple with significant practical experience
- Bilingual and knowledge of dialects is an advantage
- Passion to serve elderly and needy families
- Good team player

Responsibilities
- Evaluate outreach programmes to ensure maximum impact for beneficiaries
- Conduct and manage casework and counselling, case consultation, and home visits
- Network with relevant organizations for welfare of client, volunteer management and programme delivery
Community Care Nurse
Requirements
- Registered Nurses with local diploma or Degree qualification in nursing
- Valid license with Singapore Nursing Board
- Pleasant disposition and able to work well in a team.
- Singaporean or Singapore PR holders only
Job Description
The Community Care Nurse co-ordinates and integrates services and care for the clients.
He/She has key roles and functions which include:
- intake/screening of clients
- client engagement
- assessment
- care implementation & coordination of service delivery
- monitoring, evaluating care and outcomes
- formal reassessment and disengagement.
- duties as and when assigned by the centre manager/director.
He/She is accountable and responsible for facilitating the achievement of client based goals and applicable standards within a defined context, ensures collaboration occurs with the care team, the family and /or the client.
