Social Worker

Requirements

  • Degree holder in Social Work, Gerontological Counselling, or equivalent
  • Non degree holders also considered, provided qualifications in social work or counselling diploma related courses achieved and couple with significant practical experience
  • Bilingual and knowledge of dialects is an advantage
  • Passion to serve elderly and needy families
  • Good team player

Responsibilities

  • Evaluate outreach programmes to ensure maximum impact for beneficiaries
  • Conduct and manage casework and counselling, case consultation, and home visits
  • Network with relevant organizations for welfare of client, volunteer management and programme delivery

Community Care Nurse

Requirements

  • Registered Nurses with local diploma or Degree qualification in nursing
  • Valid license with Singapore Nursing Board
  • Pleasant disposition and able to work well in a team.
  • Singaporean or Singapore PR holders only

Job Description

The Community Care Nurse co-ordinates and integrates services and care for the clients.

He/She has key roles and functions which include:

  • intake/screening of clients
  • client engagement
  • assessment
  • care implementation & coordination of service delivery
  • monitoring, evaluating care and outcomes
  • formal reassessment and disengagement.
  • duties as and when assigned by the centre manager/director.

He/She is accountable and responsible for facilitating the achievement of client based goals and applicable standards within a defined context, ensures collaboration occurs with the care team, the family and /or the client.
